Epithelioid sarcoma with muscle metastasis detected by positron emission tomography

چکیده
BACKGROUND Epithelioid sarcoma is an uncommon high-grade sarcoma, mostly involving the extremities. CASE PRESENTATION A 33-year-old man was referred to our institute with a diagnosis of Volkmann's contracture with the symptom of flexion contracture of the fingers associated with swelling in his left forearm. Magnetic resonance imaging (MRI) showed abnormal signal intensity, comprising iso-signal intensity on T1- and high-signal intensity on T2-weighted images surrounding the flexor tendons in the forearm. Diagnosis of epithelioid sarcoma was made by open biopsy, and amputation at the upper arm was then undertaken. [18F]-2-fluoro-2-deoxy-D-glucose-positron emission tomography (FDG-PET) detected multiple lesions with an increased uptake in the right neck, the bilateral upper arms and the right thigh, as well as in the left axillary lymph nodes, with maximum standardized uptake value (SUVmax) ranging from 2.0 to 5.5 g/ml. Magnetic resonance imaging confirmed that there was a lesion within the right thigh muscle which was suggestive of metastasis, even though the lesion was occult clinically. CONCLUSION Increased uptake on FDG-PET might be representative of epithelioid sarcoma, and for this reason FDG-PET may be useful for detecting metastasis. Muscle metastasis is not well documented in epithelioid sarcoma. Accordingly, the frequency of muscle metastasis, including occult metastasis, needs to be further analyzed.

Successful Treatment with Pazopanib for Multiple Lung Metastases of Inguinal Epithelioid Sarcoma: A Case Report
Epithelioid sarcoma is a rare soft tissue sarcoma and usually resistant to chemotherapy. It has high rates of local recurrence and distant metastasis, and the prognosis after metastasis is poor. We report a case of multiple lung metastases of an epithelioid sarcoma originating in the inguinal area that we treated with the multikinase inhibitor pazopanib. The patient was a 38-year-old male who b...

thin smooth walls of varying sizes. However, the male sex prompted us to exclude a more sinister cause for the lesions [2], and upon further questioning the patient had complained of right thigh pain for several weeks. Examination revealed a fixed soft tissue mass in the mid-thigh. Histology from the thigh mass biopsy and bilateral video-assisted thoracic surgery bullectomy [3] confirmed pulmon...

BACKGROUND Pulmonary sarcomas are uncommon accounting for 0.5 % of all primary lung cancers. Undifferentiated sarcomas account for up to 20 % of soft tissue sarcomas. A lung tumor revealed to be an undifferentiated sarcoma with epithelioid features has never been reported in the literature. متن کاملMetastasis to the Skeletal Muscle from a Malignant Phyllodes Tumor of the Breast: A Case Report
Malignant phyllodes tumors are relatively rare, accounting for 0.3 to 0.9% of all tumors of the breast. This tumor metastasizes approximately 20% of the cases depending upon the histologic behavior (1). The most common site of metastasis is the lung (1). Of all possible organs, metastasis to the skeletal muscle is relatively unusual. Moreover, imaging findings of metastasis to the skeletal musc...

Marjolin's ulcers, which are epidermoid carcinomas arising on non-healing scar tissue, may be of various pathological types, including squamous cell carcinoma. The pathogenesis of squamous cell carcinoma arising in an ulcer differs from that of the primary cutaneous squamous cell carcinoma. This squamous cell carcinoma is aggressive in nature, and has a high rate of metastasis. Between January ...
